Patients served: 8,343 Subcontractors So Others Might Eat, Inc. This partnership extends Unity’s referral network for housing services as well as its reach to people in need. Unity Participates in planning and organization meetings for this coalition. in addition to Unity, the Partnership includes: the Office of the Deputy Mayor for Children, Youth, Families, and Elders Pathways to Housing Department of Mental Health Department of Health Department of Human Services and eight outreach agencies( Georgetown Ministry Center, Neighbors Consejo, Community Council for the Homeless, Unity’s mobile medical van, Rachael’s Women’s Center, Salvation Army “Grate Patrol, ” DC Central Kitchen’s “First Helping” program, and UPO Shelter Hotline vans. Case Management/Social Services, Contraceptives (issued w/counseling), Dental Care, Electronic Medical Records, Family Planning, Gynecology, HIV/AIDS screening/education, Immunizations, Labs, Mental Health Counseling, Mobile Van/Transportation, Optometry Services, Pediatrics, Physical Exams, Primary Health Care, Referrals for Specialty Care, Smoking Cessation, TB screening/education, Well Child Check-ups LinkagesĪ Community Partnership of organizations coordinates an array of housing services and community planning. Unity serves the most vulnerable residents of the District including the uninsured, working poor, homeless, the incarcerated and formerly incarcerated. Unity serves individuals and families in all eight wards of the District of Columbia through its network of health service centers that include Community Health Centers, sites serving homeless individuals and a mobile medical outreach unit. is Washington, DC’s largest nonprofit health and social services organization serving more than 81,000 individuals and families annually. Deeply rooted in the District’s neighborhoods for over 30 years, Unity strives to promote healthier communities through compassion and comprehensive primary and specialty health care and wrap-around services, regardless of the ability to pay.
